Creating affiliation form(s)
Starting with affiliation?
- On the top menu bar, select Clubs
- Select Affiliation
- On the Affiliation Forms & Processes page, click onto "Create Form" button in the Affiliation Forms table to start create affiliation forms

Creating affiliation process(s)
Once affiliation forms are created, you can create affiliation processes
An affiliation process is a group of affiliation forms with each form represents a step/stage in the process. These forms will appear as sequential steps on the clubs' portal.
- Click "Create Process" in the Affiliation Processes table (also on Affiliation Forms & Processes page)
- Fill in the process name and description
- Select a form for each stage of the process
Once a form is selected as part of a process, you cannot add the same form to other form group or process. - Mark "Forms must be submitted in order" if you want clubs to complete and have the first form approved before moving to the next one.
Making affiliation forms accessible to clubs
After you set a form to active, it will show up in the Forms section on each club’s portal. And for wider reach, feel free to share these forms on your website or social media channels.
Setting up affiliation expiration
The expiration period applies only after all submissions in the club’s affiliation application have been approved by the union. The club's affiliation status will then remain active for the set duration defined by the union. After this period, the club must re-affiliate to maintain its active status.
By default, the expiration date is set as 30 June for the US, UK, Canada; and 31 Dec for Australia and New Zealand. To change the expiration date,
- Go to Affiliation Forms & Processes page
- Under Overview, you can see the current date set for affiliation expiration. Click "Edit expiration"
- Enter the date and month of the expiration in the format MM-DD
- Select the length for the affiliation status in years
- Click Save to apply
